Regarding the requesting of official documents from Ukraine, persons living abroad apply to the diplomatic missions and consular posts of Ukraine in the countries of their residence to make a request to Ukraine.

The relevant questionnaire for retrieving documents from Ukraine should be filled out and accompanied by the cover letter, copy of any ID and a consular fee of 27 CAD (Money Order or Bank Draft payable to the Consulate General of Ukraine). 

The questionnaire must be filled out exclusively in Ukrainian in 3 copies, for each document separately. The answers to the questions should be complete and accurate.

A request to the archival institutions of Ukraine could be made independently by sending to the address of the relevant Ukrainian archive a completed questionnaire of the established sample to request a document from Ukraine. 

Diplomatic missions and consular authorities of Ukraine shall inform the person who requested the document from Ukraine about the results of the request made by a person or by the diplomatic mission/consular institution. (the processing time of such sort of requests could take from a few months to one year or more).
