Police Clearence Certificate from Ukraine

To obtain the Police Clearance Certificate from Ukraine the following documents should be submitted. To see the relevant information

- copy of the first page (with personal data) of international passport; copy of the passport page with Ukrainian visa and/or (if available) copy of the Temporary Residence Permit in Ukraine (or exact dates (period) of residence in Ukraine).

- Completely filled out and signed form anketa-dovidka-pro-nesudimist.doc.

- Money Order/Bank Draft (Certified Bank Cheque) for 41 CAD payable to the Consulate General of Ukraine. Personal cheques are not acceptable.

The documents could be mailed to 9707-110 St. NW Unit 327 Edmonton, AB T5K 2L9 and prepaid express envelope (or envelope with stamps) should be added to receive the Certificate back.
